Convert a Charting Template to an Intake Form

There may be an instance in which you'd like to switch a charting template that you have created into an intake form for clients to complete (or, vise versa). 

Navigate to Forms > Select Form > [...] > Convert to Charting Form 

if you convert an Intake Form to a Charting Form template, you will be able to select this template when creating a new charting note for a client. 

Client > Charting > Create Chart Note

From the drop-down, select your charting form template.
