Family History Details in Client Profile

As part of the ONC Certification process, Healthie now enables providers to record, change, and access their clients' family health histories with a focus on the first degree relatives' diagnoses.

To help providers appropriately document a client's first degree relatives' diagnoses, we've added an option within the Client Profile > Actions > Family History > Add Family History Entry


Family History

Within the Family History field, you'll see an "Add Family History" button. When "Add Family History" is selected, a new window for "Add Family History Entry" will appear.


Conditions

For Condition, you'll see the option to select: 

  • Alcohol Abuse
  • Alcoholism
  • Alzheimer's disease
  • Anxiety disorder
  • Arthritis
  • Asthma
  • Birth defects
  • Cancer
  • Diabetes Mellitus/Sugar disease
  • Eye and ear disorders
  • Hearing loss/deafness
  • Heart disease
  • High blood pressure
  • High cholesterol
  • Human Immunodeficiency Virus (HIV) disease
  • Kidney disease
  • Mental disorder
  • Migraine
  • Miscarriage/Stillbirth
  • Neurological Disorder
  • Other Eye Disorders
  • Parkinson's Disease
  • Schizophrenia
  • Skin conditions
  • Stroke
  • Tobacco Abuse
  • Vision loss/blindness


Relationship(s)

For Relationship(s), you'll see the option to select: 

  • Mother
  • Father
  • Sibling
  • Child
  • Maternal grandmother
  • Maternal grandfather
  • Paternal grandmother
  • Paternal grandfather
  • Other

Once the Family History item has been added, it will be displayed as below. Please note that you are able to add multiple Family History item entries on this Actions page.


Editing and Removing Family History

As a provider, in order to be able to edit and remove Family History items, clicking the three dots in the right hand of the section will present you with two options: Edit Family History Entry and Remove Family History Entry.

In order to edit your Family History, make the appropriate edits within the right hand panel that pops up, and click on Save Changes. On the other hand, to remove/delete your Family History entry, select Yes when you're asked "Are you sure you want to Remove this Family History Entry?" via pop-up.
