Duplicate (Copy) a Form

Healthie Intake Forms makes it easy to easily and electronically collect health information, e-signatures, payment details (credit cards and insurance), and other forms needed for care. 

There may be instances in which you would like to duplicate (copy) a form to serve as the foundation for another form, or to create a newer instance of the form. 

Copy a Form

You'll need to have the form open within your Forms Library in order to duplicate the form. 

Forms > Locate Form > [...] > Edit 

From here, you'll see an Options tab at the top-right of the form. 

Select Copy

Once the Form has been Copied, you'll see a green success banner. 

We recommend that you immediately Rename your form to avoid and confusion with the existing form. 

Options > Rename

This form will now be available in your forms library. You may edit it further as needed, without impacting the original version of the form.
