Using Matrix Fields in Form Builder

Healthie's Form Builder allows you to pull from many field formats, from multiple choice, to short answer, and free text. In some instances, you may find the need to use a chart, or Matrix, as it's called in the Healthie Form Builder. 


Add Matrix Field to a Form 

To add a Matrix field, you first need to start a new form or edit an existing form within the Form Builder. In the Question Bank > Default questions, you can scroll to find "Matrix" 

Drag and drop the Matrix field onto your form. You can reorder the fields as needed. 


Edit a Matrix 

Once the Matrix field is added to your form, you'll need to edit it. Click "Edit" within the form field. 

You can add/remove rows and columns to build out your Matrix. 

Add Row will add a new row below the previous one

Delete Row will remove the last row that you added 

Add Column will add a column to the right of the last one 

Delete Column will remove the last column you added

Note: You'll need to add a Row first, before you can add a Column. There is no way to delete or rearrange SPECIFIC rows or columns, so please be mindful of this as you build out your Matrix. 

Edit Field Label 

The Field Label is what the client will see as the instructions or name of the Matrix (which appears as a chart). You can edit the text and leave the field blank if you do not want anything to show. 

Row and Column Header Labels 

Just as with a typical chart, you can text into fields to add column or row labels - leaving a column or row header BLANK will prevent the header label from displaying (see example below, we've created a Matrix but left the column header above the symptoms BLANK. When viewing the Matrix, this field does not appear. Alternatively, if we had typed "Symptoms" in that field, it would appear above "Abdominal Cramping") 

Using Checkboxes or Text 

For every column that you add, you have the option of enabling the Matrix to be completed as a "Checkbox" or "Text" 

Checkbox will allow a client/provider to simply mark off an item 

Text will allow a client/provider to free text into the field. 

You can use a combination of Checkbox or Text fields within your Matrix as needed.


Viewing Your Matrix 

When you've completed your Matrix click the "UPDATE" button in order to save it, and return to the Form Builder. 

You can also Preview the entire form by going to Options > Preview - this will give you an indication of what clients may see when completing the form. 

Note: When Matrix questions are exported to a report, they are exported in JSON format. If you are not comfortable working with JSON, we suggest using another field type.
