Claim Forms: Diagnoses - Box 21 and Box 24E

When completed a CMS 1500 claim form, adding at least one diagnosis code is required. You can search in Healthie's diagnosis field for your condition. Our diagnosis codes are sourced from the ICD-10 list. ICD codes (or sometimes ICD-10) is shorthand for International Statistical Classification of Disease and Related Health Problems 10th Revision

If you do not see a code when searching the field, please email hello@gethealthie.com with the code and description you wish to add. If it is a validated ICD-10 code and billable, we will add it to our library. 


Add & Manage Diagnoses

You can add multiple diagnosis codes for a client, but only one code will be billable. You will also be prompted to add a Fist Symptom Date for each condition. Once this information is added into a client's claim form once, it will automatically populate on subsequent claim forms, saving time on claim creation & improving claim accuracy. The diagnosis details will also update in the Client's Profile (learn more here). 

1. In "Form Builder", CMS 1500 form, and Client Profile, the "Diagnosis" field in the "Client Info" bank includes the following:

  • 1CD-10 Search Field 
  • Active checkbox
  • Onset Date (First Symptom Date) 
  • End Date

2. When a diagnosis is set to active, the end date field is automatically disabled.

3. Providers can dynamically add more diagnosis codes by clicking "Add Diagnosis"  

4. Pressing the "Delete Icon" will remove the diagnosis.


Diagnostic Pointers

When completing the Billing Items of the form section, you'll be asked to make a Diagnostic Pointer selection. The billing code needs to point to the diagnosis code that you will be billing for. 

In this example, Diagnostic Pointer 1 matches with Diagnosis 1 listed in the section above. 

On the CMS claim form, diagnoses populate Box 21 A - J. The first diagnosis you listed in Healthie will always populate the 21A spot on the claim form.
