Outcome Measures Forms (Free) 

Outcome measures are standardized forms that are designed to measure the current condition of your patients. These tools are meant to give an objective look into how your patient's condition is affecting different parts of their everyday life. 

Lower Extremity Functional Scale (LEFS): 

A questionnaire that assesses the current functional limitations of patients with diagnoses affecting their lower extremity. 

Oswestry Disability Index (ODI): 

A questionnaire that focuses on the impacts that low back/leg pain has on a patient's ability to perform functional activities and the severity of their condition. 

NDI: Neck Disability Index (NDI): 

A questionnaire that focuses on the impacts that neck pain has on a patient's ability to activities in their daily life and level of disability.

DASH: Disability of the Arm, Shoulder and Hand Questionnaire: 

A questionnaire for conditions of the upper extremity, that focus revolves around level of disability and changes in function over time. 

Hip Disability and Osteoarthritis Outcome Scale (HOOS): 

A questionnaire that measures the impact that a diagnosis involving the hip can have on a patient's ability to perform daily activities, pain levels and quality of life.

Outcome Measure Forms (Possibly For a Fee)

There are outcome measures that have fees associated with them. These are not directly available on our website and will need to be purchased by the provider themself. However, once purchased you can send us a receipt and we can give you access to build out the forms on your Healthie account. 

The BERG Balance Scale: 

Assesses a patient's static balance and how at risk they are for falls.

The Dynamic Gait Index (DGI): 

Assesses a patient's dynamic balance, specifically, the effects that external demands have on their balance while walking.

A questionnaire that assesses a patient’s quality of life.
