Auto-Score Forms

Healthie's Form Builder supports the creation of intelligent and versatile forms with auto-scoring functionality. 

Member's on Healthie's Enterprise Plan can request a form with auto-scoring functionality be built & added directly to your account. 

To request an auto-scored form be added to your account, you can send a copy of the form to hello@gethealthie.com.

• Accepted formats: PDF (strongly preferred), Word Doc, Google Doc, or Typeform Link. Photocopies are not accepted due to legibility issues. 

Once received, the Healthie Support team will review your form and be in touch for any clarifications or adjustment that may be needed. 

Notes on Auto-Scoring: 

• Healthie can set up forms to auto score but can only take sums of answers, can't do custom calculations.
• In order for auto scoring, the answer choices need to have the numerical value included in the summation as the first character in the string of text.
• GAD-7 and PHQ-9 Auto-Score forms are already available to you within your Healthie Forms Library.

• Specifically for matrix fields, formatting of an Auto-Score Form needs to be 2 columns with the second column’s values as Text Boxes. Please note that checkboxes will cause the form to throw an error when viewing it as a completed form.

  Additionally, the score from one individual matrix field cannot be split up to into different subtotals, all score numbers in the matrix field have to go into one subtotal.

  For example, in the following screenshot, the three Score fields will be added together and then added into whichever Score total they belong to.
