COVID-19 Screening Form

When working with clients in-person, it's important for healthcare providers to properly screen for any clients that may have been exposed to COVID-19. 

A new form has been added to Healthie's internal form library: COVID-19 Screening Form. This form will be accessible to all providers by navigating to Forms > Search your Forms Library. Add this form to your intake flow or share with any clients prior to an in-person visit for screening purposes. 

You can edit this form by locating it within your forms library, and opening the form. Modify any question, add your own questions, add your logo and business information within the header, and more.
