Advanced Beneficiary Notice Form

A new form has been added to Healthie's internal form library: Advanced Beneficiary Notice (ABN). Also known as a waiver of liability, an ABN is a notice provider should give to clients before they receive a service if, based on Medicare coverage rules, you believe Medicare will not pay for the service.

This form is accessible to all providers by navigating to Forms > Search your Forms Library. Add this form to your intake flow or share with any clients prior to an in-person visit for screening purposes. 

You can edit this form by locating it within your forms library, and opening the form. Modify any question, add your own questions, add your logo and business information within the header, and more.
