Have Clients Complete a Healthie Form
Healthie's makes it easy to automatically and electronically collect health information, e-signatures, payment details (credit cards and insurance), and other forms needed for care.
Once completed, forms are automatically stored with a client's profile, for providers to access or view at any time. If you have not done so already, you'll want to build and customize your intake forms.
This article reviews ways to send forms to clients (manually or automatically) for clients to electronically complete.
Create Intake Flow(s)
Create an intake flow to automatically request clients complete a set of forms (ie. during onboarding). Intake flows are tied to client groups, and are triggered when a client is added, or moves, into a new group.

Set Recurring Forms
Set a cadence for the same form to be completed by a client on a periodic basis by setting the form as a recurring form.
Tie Form(s) to Appointments
Send a form after a session is booked and/or completed (ie. to collect post-session surveys).
Send a One-Time Form Request
Request that an existing client complete a form, and how to send a reminder for a client to fill out the form. If they have already completed the form in the past, sending the new form request will prompt them to complete the form again (ie. to collect updated information from the client, such as insurance details).
