Add a video to an intake form

Healthie's Intake Forms are designed to be customizable and branded for your business. Many providers choose to add a pre-recorded video to their form(s), for clients to view - such as a Welcome Video. 

To add a video to an existing form or form template: 

• Navigate to the Forms tab > Select Form > Change question bank from Default to Video 
• From the Question Banks drop-down, select "Video" > a video field will appear. 
• Drag this field to the right, and drop it into your form. 
• Click "EDIT" at the right of your field.
• Paste the URL link for your video. You cannot upload the video file directly, you must first upload it to a video hosting platform (Vimeo, Loom, Youtube) and copy the sharing URL link. 

Your video will automatically embed within your form. Click "Preview" on the top right to preview your form, as your clients will experience it.
