Add a video to an intake form

Healthie's Intake Forms are customizable and brandable, giving you the flexibility to add engaging content—like a welcome or onboarding video—to enhance your client experience.

Walk through how to add a video to a Healthie form:

Add a Video Field to a Form

• Navigate to the Forms tab > Select Form
• From the Question Banks drop-down, select Video
• Drag this field and drop it into your form

Once added to your form, you'll see [...] with the drop-down option to Edit the video field.

• Paste the URL link for your video. You cannot upload the video file directly. You must first upload it to a video hosting platform (Vimeo, Loom, Youtube) and copy the sharing URL link. 

Your video will automatically embed within your form.
