Request Clients Upload an Image While Completing a Form

Healthie's Intake Form feature makes it easy to automatically and electronically collect health information, e-signatures, payment details (credit cards and insurance), and other forms needed for care. Providers and businesses can either leverage Healthie's library of default forms, or build their own using our intuitive form-building tool. Providers can also build images into a form, including logos. 

In some instances, it may be helpful to request a client upload an image, ie. of themselves, a form of identification, or of something else relevant to their care. Healthie offers several default forms that can capture an image: 

• Client Profile Picture: to prompt a client to upload an image for their Healthie account
• Client Photo ID: to capture the front & back of a client's identification
• Client Insurance Form: to capture the front & back of a client's insurance card

The File Upload field is available to Enterprise members and can be requested by emailing hello@gethealthie.com. Once enabled, a File Upload option will appear in the Form Builder and can be dragged onto any intake form.

As an alternative, we recommend that clients use the Documents feature to upload an image file and share it with their provider. Learn more: Clients Viewing and Uploading Documents
